I recall using one called Telepin (black & pink logo). You could also just get a calling card (scratch-card type or print-out) and use that from any landline. Then get a topup voucher at the same place (it will usually just be a print-out from the register with a special top-up code number). It should cost 5 PLN for the SIM though they also have a 9 PLN version (no difference, just more credit on the card). Remember to get the "Red Bull na kartę" offer which is the prepaid one. I would try at any newsagency/kiosk/convenience store.
